For the provider who applied or prescribed the wrap. One submission per patient case. Please do not enter any patient identifying information.

What happens to your answers

Short version: they stay anonymous, a person reads them, and they change the product before it ships.

They stay anonymous

The patient survey asks for no identifying information. Clinicians enter their own reference code for a case, never a patient name or record number.

They reach the product team

Responses go to the people who design and test Ravi-Wrap™. Any report of a skin reaction or adverse event is routed for follow-up the day it arrives.

They shape what ships

Comfort, fit and outcome feedback informs the product ahead of release — which is the whole point of asking before, rather than after.

Ravi-Wrap™ is a product of Sun Scientific, Inc. Responses are used for internal product evaluation and may inform future materials.
